JOB SUMMARY AND ESSENTIAL FUNCTIONS: Provides social work services and counseling for patients and their families in close conjunction with the physician and other health care team members.

Provides timely referrals as appropriate for further psychosocial assessment.


EDUCATION: Bachelor Degree in Social Work or related field from an accredited college or university.

Qualified by education, training or experience to work with the adolescent, adult and/or geriatric patient population as specialty assignment dictates.

EXPERIENCE: One (1) year of social work experience preferred.

An ability and willingness to demonstrate and maintain clinical competency as required for area of specialization.


Ability to determine different needs of patients/families

Ability to interact with other professionals in a team framework

Excellent verbal and written skills

Ability to establish constructive relationships with resistant clients by overcoming strong initial resistance or dealing with conflict-laden or complex situations.

A thorough knowledge of several types of vulnerable populations, corresponding service delivery systems, methods for assessing needs and planning to relate the service delivery systems to human needs that are high priorities

Maintains knowledge of Federal and State (Medicaid, TNCare, and Medicare) regulations

An ability to manage a sometimes chaotic work environment.

Ability to modify and update plans/interventions, based upon changing patient needs

Ability to work with patients and families with acute, chronic, and complex disease processes, and those who are dying
