Overview

The Community Outreach Coordinator will develop an outreach and marketing plan outlining how best to accomplish the goals and objectives of the PACE marketing initiatives.

The Community Outreach Coordinator is part of a diversified team of professionals all working towards serving a common goal – helping seniors in our community “Live Life Their Way”

Valir PACE is a Program of All Inclusive Care for the Elderly (PACE), and is the future of healthcare. It utilizes a team-based care approach providing a personalized, comprehensive healthcare plan to the most at risk, those without access to quality healthcare, the under and uninsured. Individuals enrolled in our program have better health outcomes, less visits to the emergency room, while continuing to live in their own home, often times without ever having to go into a nursing home.

The focus of every PACE organization is to help the elderly live in the community for as long as possible. To meet this goal, PACE organizations focus on preventive care. Although all people enrolled in PACE are eligible for nursing home care, only seven percent live in nursing homes.

The ideal Community Outreach Coordinator is an outgoing, compassionate person that wants to make a difference. The perfect candidate will have the ability to wear many hats allowing them to build relationships with case managers, physicians, administrators, potential participants and their families. The Community Outreach Coordinator will give tours of the center to family members and participants interested in the program, as well as potential referral sources, as necessary. House visits to qualified leads, as well as taking a participant through the entire enrollment process will be the responsibility of the Community Outreach Coordinator.

Qualifications

POSITION REQUIREMENTS:

Licenses or Certifications: Valid Oklahoma Driver’s License

Total Education, Vocation Training, and Experience: Bachelor degree in a related field or 5 years relative experience.

Skills and Abilities:

Ability to work independently under general instruction on a variety of special projects;

Ability to perform critical thinking, active listening, judgment and decision making, effective speaking, time management, persuasion, persistence, flexibility, and social perceptiveness;

Excellent oral/written communication skills;

Ability to interact in a positive manner and communicate well with staff, referral sources, and the community about services provided by Valir Health and the company’s mission, philosophies, goals, and objectives.

Ability to perform computer skills.
